Irregular Periods After 30 — Is It PCOS, or Something Else?

Irregular Periods After 30 — Is It PCOS, or Something Else?

A cycle that was reliable for years suddenly starts arriving early, late, or not at all — and the first search result is almost always PCOS. It's a reasonable guess, since PCOS is common. But it's not the only explanation for irregular periods after 30, and treating it as the default answer can mean the actual cause goes unaddressed. UHS Vista's gynaecology department combines a same-visit consultation with in-house hormone testing and ultrasound to settle the question properly.

Why PCOS Gets Blamed First

Polycystic Ovary Syndrome affects a significant share of women of reproductive age and often first becomes noticeable through irregular or missed cycles, along with symptoms like acne, excess facial or body hair, weight gain concentrated around the abdomen, and difficulty conceiving. Because it's common and well-known, it tends to be the first thing people — and search engines — point to.

What Else Can Look Like PCOS but Isn't

  • Thyroid disorders — both an underactive and overactive thyroid can disrupt cycle regularity, and are diagnosed with a simple blood test rather than an ultrasound
  • Perimenopause — for women closer to their late 30s and 40s, irregular cycles can be an early sign of the transition toward menopause, often years before periods stop entirely
  • Stress and significant weight change — both can suppress or disrupt ovulation temporarily without any underlying disorder
  • High prolactin levels — an elevated prolactin hormone can cause irregular or absent periods and is easily missed without the right blood test
  • Uterine fibroids or polyps — these tend to cause heavier or longer periods alongside irregularity, rather than purely spacing-out of cycles

The symptom overlap is exactly why self-diagnosing from a symptom list rarely settles it — several of these conditions share two or three signs with PCOS while needing completely different treatment.

How a Gynaecologist Actually Tells Them Apart

Detailed Cycle History How irregular, for how long, and what other symptoms are present.

Physical Exam and Blood Work Hormone panels, thyroid function, and prolactin levels — PCOS itself is diagnosed on a combination of findings, not on irregular periods alone.

Pelvic Ultrasound, If Needed Looks directly at the ovaries and uterus to check for cysts, fibroids, or polyps.

Why It's Worth Sorting Out, Not Just Managing the Symptom

Irregular cycles left uninvestigated for years can delay treatment for conditions that affect fertility, bone health, or long-term metabolic risk — PCOS and thyroid disorders both fall into this category. Getting an actual diagnosis, rather than assuming and self-managing, changes what treatment looks like and how effective it is.

When Should You See a Gynaecologist for Irregular Periods?

  • Cycles have been irregular for more than 2–3 months
  • Irregular periods alongside acne, excess hair growth, or weight gain
  • Periods have become heavier, longer, or more painful than before
  • You're trying to conceive and cycles are unpredictable
  • Periods stop unexpectedly before the usual age for menopause

Visit UHS Vista for a Gynaecology Consultation

Irregular periods after 30 are worth investigating rather than assuming, since PCOS, thyroid disorders, and perimenopause can look nearly identical from the outside. A consultation with the right blood work usually settles the question, and what treatment should look like, within a visit or two.
